Pre-k Registration Instructions:

 

1. For Pre-K Registration for the 2021-2022 school year, please click on the above link and select year 2021-22

Pre-K Registration for the 2022-2023 school year, please click on the above link and select year 2022-23

2. Visit the Early Childhood webpage to download and complete the listed documents.  You will need to email the documents to the registration personnel at the identified school:

  1. Registration application
  2. Emergency contact form
  3. Health history form
  4. Medical history

3. Important information to remember:

 

Child’s birth certificate documenting the following:
For 2022-23
Pre K 3 - Must be 3 years old on or before Oct. 1, 2022
Pre K 4 - Must be 4 years old on or before Oct. 1, 2022
For 2020-21
Pre K 3 - Must be 3 years old on or before Oct. 1, 2021
Pre K 4 - Must be 4 years old on or before Oct. 1, 2021
 
Documents needed for registration:
 
Immunization record
 
Health history form (to be completed by parent/guardian. Can be found on the Early Childhood registration web page)
 
Medical examination form (to be completed by a pediatrician or health care provider. Can be found on the Early Childhood registration web page.)
 
Two (2) current proofs of residency (Please see list below):
           New Jersey Driver’s License
           State identification
           Utility bill (example: PSE&G, SUEZ Water)
           Telephone bill
           Cable TV bill
           Residency evidence on leasing company's letterhead
           Employment documents
           Financial account information
           Benefit statements
           Notarized letter from private homeowner including names of all persons residing in the residence
           Unemployment claims
